Camper Application Process

Our goal is to accept as many campers as possible to camp each summer. In order to do so, we need to know the ins-and-outs of each child's health history to ensure we can provide the physical and emotional support they require. Therefore, we have a rather extensive application and review process which is outlined below.

Step 1: Apply

To get things started, fill out the online application in CampDoc. This creates a record in our system of your child's interest in attending either our June or July session. It also contains basic info like age, gender, and parent/guardian contact details. Access to this site will be required on a recurring basis, so save your login and password. Once the application is complete, immediately proceed to step 2 and complete the required health profile.

step 2: Forms

Once an application has been submitted, it's time to complete the required health profile information. Dependent on whether the applicant is a patient or sibling, this will consist of either one or two steps: all applicants must fill out the online Health Profile in CampDoc, while only patients on-treatment need to also submit a Medical Sign-Off Form signed by a physician or nurse practitioner saying the child is approved to attend camp.

step 3: Acceptance

Applicants are not guaranteed acceptance to camp until they receive an official acceptance email from the camp director. This will take place after the application deadline has passed and the director and medical team have had an opportunity to review both the health profile. If applicants meet our requirements, we can safely care for the child, and we have an open space, we will happily approve their application and invite them to camp.


Application Timeline

Applications are only live (available for submission) during a specific period of time, generally from February–March/April. Applications filled out outside of the current year's open period will be placed on a waitlist.

 

Health and safety Information

The following is a partial list of the types of information that you will be asked to supply as part of the application process. Please have it readily available to quicken the pace of your application. Once input, the information will be saved for the following year's application.

  • Immunization history (month and year)

  • Medications (name, dose, time of day administered)

  • General health history (height and weight, former surgeries, learning disabilities, etc.)

  • Allergies

  • Activity restrictions

  • Healthcare provider and insurance information (including image of insurance card)

  • Emergency contact information

  • Approved persons to pick child up from camp
